Social Care Programme
The social workers inform and advise on the processing of public and private social resources for all users.
These are the main activities they carry out:
- Advice on the application or review of the degree of dependency.
- Advice on the application or review of the degree of disability.
- Information/Management of the AA Card
- Advice on the application for the ‘Programa Acomp谩帽ote’ (Accompany yourself programme) for GPS tracking devices.
- Advice on applying for the ‘Family Respite Programme’ for carers of dependent persons.
- Advice on applying for a parking card for people with disabilities.
- Referral to socio-community resources.
- Advice on applying for Bonotaxi (Taxi voucher)
Three types of interventions are carried out:
1. Information.
The social worker provides initial attention to the relatives or carers of the person diagnosed with some type of dementia. By means of a face-to-face appointment, the information and services available at the centre are explained to them, as well as the socio-community services available to them.
2. Referral
There are two types of referrals:
Internal: services developed by the association
- Psychological support
- Legal support
- Information on support products
- SAD
- Cognitive stimulation service at home
- Physiotherapy at home
- ‘Cultiva a Mente’ Prevention of cognitive decline
- Training programme
External: services of the public administration, private centres or other entities
- Primary care social services
- Specialised doctor
- Residence
- SAD
- Public day centre
- Private day centre
3. Social care
The associated family and member of AFAGA can request information about social and/or private resources, as well as resources available in the community.
